Please note that your completed questionnaire will be treated with the necessary confidentiality.  Upon completion, a copy of this form will be sent to you and the elder who leads your congregation who may, in his discretion, distribute it to the local eldership team in your congregation. From there, the form will go to Andrew who, in his discretion, may also distribute it to the elders who direct the affairs of the church.

Just a reminder however, that completion of this questionnaire in no way commits you, or us, to any future decisions regarding your role or ministry within JoshGen.
